Appointments
Q: How do I schedule or change an appointment?
You can contact our front desk or message us through the OnPatient Portal.
We ask for at least 48 hours' notice for cancellations or reschedules.

Q: Are appointments in-person or virtual?​
All appointments are currently virtual until further notice. You can join from your phone, tablet, or computer using your telehealth link.
Telehealth
Q: How do I join my telehealth appointment?
Check your email or text message for your secure link, then click to join at your scheduled time. Make sure your camera and microphone are on.

Q: What if I can’t connect?
Try refreshing your browser or restarting your device. If you still can’t connect, please contact our front desk and we’ll help you right away.
New Patients
Q: How do I become a new patient?
Start by completing our New Patient JotForm on the “New Patient Info” page. Once submitted, we’ll send your consent forms via email.
Q: Do I need to sign up for OnPatient right away?
Yes — after completing your new patient form, you’ll receive an invitation to create your OnPatient Portal account to manage forms and messages securely.

Q: Which insurances do you accept?

Q: How do I contact my provider between sessions?
